The evidence to bring
- Intake and assessment forms capturing cultural, religious, gender and other identity preferences at the participant's direction
- Support plans showing how those preferences shape delivery (worker gender, food, observances)
- Cultural awareness or diversity training records for workers
What assessors commonly find
- Cultural and identity questions skipped or completed by staff without asking the participant
- Preferences recorded at intake but not reflected in rosters or plans

Answer the reviewer first
Show me intake and assessment forms capturing cultural, religious, gender and other identity preferences at the participant's direction, and tell me who owns it and when it was last reviewed.
That is the question a quality assessor opens with for this requirement. Have the record on the desk and the date it was last reviewed to hand.
